I say it's a class issue. People from higher social classes are less likely to be overweight simply because it's seen as low class. And furthermore, people from higher social classes are also more health obsessed in general so are more likely to make sure their children brush.

The amount of money available to buy food.
The time available to cook meals.
Poor people are less able to afford fresh fruit and veg, when a "meal" of McD or BK costs only $5 per person.
If both mum and dad work, preparation and cooking time of an hour reaches a significance when McD or BK can provide in 5 minutes.
